Use the same words
A diaper log can be as simple as a time and the words wet, dirty, or both. Agree on those words with anyone who helps with changes. Short, clear notes are easier to read when you're tired.
Write what you noticed during that change. This guide is about keeping notes, not deciding how many changes your baby should have. Ask your care team about health questions or anything that worries you.
One change may have two notes
A made-up example is a diaper changed at 9:40 AM that was both wet and dirty. On paper, you could write '9:40 AM – both.' If your app has separate wet and dirty choices, keep the two notes tied to that same change.
Swaddlely offers separate Wet and Dirty quick-log options. Two entries can describe one diaper change; they don't always mean two separate changes. Keep that in mind when you read back through the times.
Avoid adding the same change twice
Choose one person to record a change. If you ask your partner to add it, let them know you haven't already done so. That small check can save you both from trying to fix the record later.
If two people do add the same event, compare the times and ask what each entry meant. Don't delete a note just because the times are close. It may describe a different change.
Make the record easy to find
Keep paper notes in one place or use a shared log your helpers can open. Show each person where to look before they start adding notes. Make sure they're viewing the right child.
If you missed a time, say it was approximate or unknown. The point is to leave a useful note for the next person, not to make every line look perfect.
Check two close entries before counting them
Close times are a clue, not proof of a duplicate. A wet note and a dirty note at 9:40 AM may describe one change. Two wet notes close together could be a duplicate or two separate changes. Ask the people who recorded them before deciding.
For a paper log, give each change one row and mark the observations in that row. If separate app entries describe the same change, keep that relationship clear in your notes. Raw entry counts are not always counts of changes.
